Waiver of Liability

BY SIGNING this form, each player and parent/GUARDIAN CERTIFIES THAT HE/SHE has read, understands and accepts this waiver. I waive, discharge and release,the 2005 Utah College Showcase, its game directors, sponsors, coaches, employees, AGENTS, other players, practice venues, Lehi High School, Alpine School District, and The City of Lehi, Utah from all actions, suits, claims, and demands whatsoever in law or inequity, including, but not limited to, ANY physical injury and/or career ending injury,and ANY damage TO or loss of personal property while participating in the 2005 Utah College Showcase at ANY venue.
