The following is needed to register each player for the team. Please bring these items on Thursday June 5th.

1. Registration form
2. Parent Code of Conduct form
3. Copy of birth certificate
4. photo
5. Check made to NUCS for $110
6. Later this summer an additional $40-$50 will be due to pay for uniform
